This checklist helps us understand some of the true underlying causes behind behaviour. When we identify areas that may require more understanding, we can provide preventative measures to avoid high risk behaviour.

CARE PROFILE

Narrowed down to relevant information needed to manage behaviour at a glance. One page front and back, filled out by family and given to care teams and caregivers to assist in de-escalating behaviour and further understanding the person on a social level. The exact answers required to manage behaviour when you don’t exactly know the person. Medication logs included, as requested.
