Developed by skilled professionals with expertise in advanced care planning for long-term care facilities, this service aims to ease the transitions that come with dementia. Whether adjusting into long-term care, or taking a break with respite care, our comprehensive plans offer valuable insights for facility staff. Our service extends to benefit caregivers entering your home, providing them with the knowledge and tools necessary to deliver compassionate care.
TERMS AND CONDITIONS
- We require a brief 10-15 minute video conference (your camera does not need to be on) but you must have a working microphone so we can clarify necessary details. This is to ensure that the care plan will reflect the most accurate and up-to-date information as possible.
- By presenting this care plan to the facility or staff, the (Substitute decision-maker, Power of Attorney) SDM/POA/Caregiver acknowledges that he/she assumes all responsibility for the direction of implementing, or not implementing these recommendations.
- The recommendations outlined are intended solely to enhance compassionate care, they are not medical in nature, and we must emphasize that any medical conditions, treatments and advice should be addressed with a qualified physician.
- Caring for Dementia stresses the importance of the SDM/POA/Caregivers role in overseeing and ensuring the well-being of the individual, we do not provide revisions to the care plan after 72 hours.

What is an advanced care plan?
An advanced care plan is a written document that outlines an individual’s preferences and instructions for their daily care, and how to maintain their overall well-being. It helps ensure that the person receives personalized and appropriate care based on their values, wishes, and goals, even when they may no longer be able to communicate or make decisions independently.
What Does it include?
Our Care Plans include a personal information/emergency contact header, and personalized strategies for 7 key areas:
- Personal Hygiene
- Mobility
- Effective Communication
- Nutrition and Hydration
- Behaviour Support
- Day and Night Routines
- Activities and Hobbies
* Also included are evidence-based “educational enhancements”. These are generalized strategies that work well with the general dementia population.
